The second programinvolves five of the state`s largest health insurers and the State of Colorado,along with a unique financial reimbursement model that rewards practices fortransforming into medical homes and, as a result, producing better outcomes forpatients. The first program will integrate the medical home concept into the curriculumand practices of all the state`s family medicine residencies, making it thenation`s most comprehensive statewide effort of its kind.
The two programs are being coordinated by CCGC, a Lakewood, Colo.-basednonprofit focused on improving health care in Colorado, along with severalpartners. The goal of the Medical Home is to provide patients with easyaccess to a personal health care team that knows who they are, what theirhistory is and are dedicated to helping them navigate our complex health caresystem when they`re sick and more importantly, work with patients pro-activelyto stay healthy. The patient-centered medical home concept is an innovative approach to healthcare that provides patients with comprehensive, coordinated care through aprimary care team.
